Abstract
Background: Acute gastroenteritis (GAE) is a major cause of morbidity and mortality in children. Diarrhea, the primary clinical manifestation of AE, carries the risk of dehydration, requiring comprehensive nursing care, including the use of complementary honey therapy.
Purpose: To describe the application of nursing care through the administration of honey to reduce the frequency of diarrhea in pediatric patients diagnosed with acute gastroenteritis (GAE).
Method: This nursing care activity uses a case study approach, encompassing assessment, establishing a nursing diagnosis, planning interventions, implementing interventions, and evaluating outcomes. The activity was conducted in the Ade Irma Suryani Pavilion Room, 2nd floor, Gatot Soebroto Army Hospital, from January 20–22, 2026. The study subjects were pediatric patients diagnosed with acute gastroenteritis (GAE), characterized by increased frequency of loose, watery bowel movements without severe complications. The objectives of this study were to reduce the frequency of diarrhea and improve stool consistency in pediatric patients with AE. The intervention consisted of administering 5 ml of pure honey three times a day for 24 hours as a complementary therapy alongside standard medical therapy. Data collection included observing bowel movement frequency, assessing stool consistency using the Bristol Stool Scale, and monitoring general condition and signs of dehydration. Data were described by comparing the patients' condition before and after honey administration to assess changes in diarrhea frequency and stool consistency.
Results: The patient experienced diarrhea with a frequency of 4–5 times per day with a liquid, mucus-like consistency without blood, accompanied by vomiting three times, decreased appetite, fluid intake of less than 1 liter per day, and the child appeared weak. Physical examination showed sunken eyes, dry lips, slow return of skin turgor (±2 seconds), weak pulse, and weight loss from 14.5 kg to 13.70 kg. Fluid balance showed a negative result of -223 ml/24 hours. Stool consistency based on the Bristol Stool Scale was type 7 (liquid), and bowel sounds were heard to be increased. Routine stool examination showed positive fungi indicating an imbalance in intestinal flora (dysbiosis) and positive fiber indicating increased intestinal peristalsis. Based on the intervention results table, on the first day the frequency of diarrhea was still 3-4 times per day with stool consistency type 7 (liquid), on the second day the frequency of defecation decreased to 1 time per day with consistency type 6 (liquid mixed with dregs), and on the third day it remained 1 time per day with consistency type 4 (long, soft, and smooth).
Conclusion: A three-day honey intervention for pediatric patients diagnosed with Acute Gastroenteritis (AEG) resulted in a decrease in bowel movement frequency accompanied by a gradual improvement in stool consistency. The application of nursing care combined with honey therapy provided a positive clinical response in helping reduce the frequency of diarrhea in children with acute gastroenteritis.
Suggestion: Honey administration can be considered as an adjunct therapy for children with acute gastroenteritis, as recommended by healthcare professionals.

Pendahuluan: Gastroenteritis akut (GEA) merupakan penyebab utama morbiditas dan mortalitas pada anak. Diare sebagai manifestasi klinis utama GEA berisiko menyebabkan dehidrasi, sehingga memerlukan asuhan keperawatan komprehensif termasuk pemanfaatan terapi komplementer madu.
Tujuan: Untuk menggambarkan penerapan asuhan keperawatan melalui pemberian madu dalam menurunkan frekuensi diare pada pasien anak dengan diagnosis Gastroenteritis akut (GEA).
Metode: Kegiatan ini merupakan asuhan keperawatan dengan pendekatan studi kasus yang mencakup tahap pengkajian, penetapan diagnosis keperawatan, perencanaan tindakan, pelaksanaan intervensi, serta evaluasi hasil. Kegiatan dilaksanakan di Ruang Paviliun Ade Irma Suryani lantai 2 RSPAD Gatot Soebroto pada tanggal 20–22 Januari 2026. Subjek penelitian adalah pasien anak dengan diagnosis medis Gastroenteritis Akut (GEA) yang ditandai oleh peningkatan frekuensi buang air besar bertekstur cair tanpa disertai komplikasi berat. Sasaran dalam penelitian ini adalah penurunan frekuensi diare dan perbaikan konsistensi feses pada pasien anak dengan GEA. Intervensi diberikan berupa pemberian madu murni dengan dosis 5 ml selama 3X24 jam sebagai terapi komplementer bersamaan dengan terapi medis standar. Pengumpulan data dilakukan melalui observasi frekuensi buang air besar, penilaian konsistensi feses menggunakan Skala Tinja Bristol, serta pemantauan kondisi umum dan tanda-tanda dehidrasi. Data digambarkan dengan cara membandingkan keadaan pasien sebelum dan setelah pemberian madu untuk melihat perubahan frekuensi diare dan konsistensi feses.
Hasil: Pasien mengalami diare dengan frekuensi 4–5 kali per hari dengan konsistensi tinja cair berlendir tanpa adanya darah, disertai muntah sebanyak tiga kali, nafsu makan menurun, asupan cairan kurang dari 1 liter per hari, serta anak tampak lemas. Pemeriksaan fisik menunjukkan mata tampak cekung, bibir kering, turgor kulit kembali secara lambat (±2 detik), nadi teraba lemah, dan terjadi penurunan berat badan dari 14,5 kg menjadi 13,70 kg. Balance cairan menunjukkan hasil negatif sebesar –223 ml/24 jam. Konsistensi feses berdasarkan Skala Tinja Bristol berada pada tipe 7 (cair), dan bising usus terdengar meningkat. Pemeriksaan feses rutin menunjukkan jamur positif yang mengindikasikan adanya ketidakseimbangan flora usus (dysbiosis) serta serat positif yang menunjukkan peningkatan peristaltik usus. Berdasarkan tabel hasil intervensi, pada hari pertama frekuensi diare masih 3–4 kali per hari dengan konsistensi feses tipe 7 (cair), pada hari kedua frekuensi buang air besar menurun menjadi 1 kali per hari dengan konsistensi tipe 6 (cair bercampur ampas), dan pada hari ketiga tetap 1 kali per hari dengan konsistensi tipe 4 (berbentuk panjang, empuk, dan halus).
Simpulan: Intervensi pemberian madu pada pasien anak dengan diagnosis medis Gastroenteritis Akut (GEA) selama tiga hari menunjukkan penurunan frekuensi buang air besar disertai perbaikan konsistensi feses secara bertahap. Penerapan asuhan keperawatan yang dikombinasikan dengan terapi madu memberikan respons klinis yang positif dalam membantu menurunkan frekuensi diare pada anak dengan gastroenteritis akut.
Saran: Pemberian madu dapat dipertimbangkan sebagai terapi pendamping pada anak dengan gastroenteritis akut sesuai anjuran tenaga kesehatan.
